The Following Additional Paperwork is REQUIRED by the GLL.

Players with incomplete paperwork will NOT be allow to participate in any sanctioned activities (including practice) until this paperwork is submitted and approved by the GLL.

Thank you for registering your child to play Lacrosse with the Grayson Athletic Association.  The GLL requires several additional items be included for the registration of each player.  All forms and paperwork MUST be submitted no later than January 8, 2017.  In the past we have had a mad dash to finish up forms at the end, if you have any questions please contact us.  The following items MUST be completed and/or submitted online:

  • GLL eForm - this includes the GLL registration form, the code of conduct, parental consent, and concussion awareness form.  Please be sure to use the players legal name as it will appear on the Proof of Grade and Physical Examination Form.  The 2017 Form is available below.

Change for 2017:

In order to complete the GLL forms and submit proof of grade and physicals, you must go to the GLL Website and register.  This will allow you to add information about your child as well as upload the forms in one central location as that information becomes available.

 After registering you will be able to add your child to your registration and complete the necessary forms and upload your proof of grade and physical forms directly on the GLL Website.  Remember that sports physicals are valid for one year from the date of issuance.  It is important that your physical will be valid over the entire duration of the Spring 2017 Season.  All GLL forms can be completed and submitted by clicking 2017 GLL Sports Physical.

VERY IMPORTANT:  We will NOT be able to upload files for you this year, you have to upload the files through the account you will create on the GLL Website.  if you do not have access to a scanner, take a clear and legible photo of the document and upload that.

*The GLL Physical Examination Form (new form for 2017 can be downloaded HERE) must be completed and signed by a Physician, Physician’s Assistant, or Registered Nurse (an LPN is NOT accepted).  If your child has a sports physical from a previous sport (such as GAA Football) that is dated after June 1, 2016 please fill out the parent portion of the GLL Physical Form and attach the previously completed sports physical to the GLL Physical Form  These items MUST BE SCANNED and uploaded using the links above..

 **Proof of Grade can be a Report Card or parent portal printout from the current school year.  You may mark out your child’s actual grades if you wish, but the grade report must include information sufficient to indicate your child’s grade for the 2016-2017 school year  Proof Of Grade MUST BE SCANNED and uploaded using the links below.

We have lots of folks in our lacrosse community that can help with getting these forms submitted.  We will not be able to accept the burden of scanning documents as we did in previous years because that is a job in an of itself.  You can even take a (CLEAR) picture using your cellphone and attach that for the Physical and Proof of Grade!

Important Lacrosse Dates
